Course Attendees Please Note
As of September 20, 2004, there will be a charge of $25 plus shipping and handling for all TOTA continuing education replacement certificates. This applies to TOTA-sponsored courses only, not to TOTA Approved Providers. Attendees must contact the appropriate Approved Providers for replacement certificates for their courses.
Program Description
For information about the Texas Occupational Therapy Association approved provider program, download the Program and Criteria in pdf version. Please read the program guidelines in detail before submitting an application. Your application and signature indicate that you have agreed to adhere to the program guidelines. Failure to adhere to the program guidelines can result in revocation of the Approved Provider Status.
Fees
The fee of $500.00 must be included with the completed approved provider application. Application and fee should be mailed with your check to the TOTA office at the PO Box listed below or faxed to the TOTA fax number. If faxing your application, be sure to include your full credit card number and expiration date where requested on the application.
Approved provider status is effective for the two year period. To apply, download a Word Document version of the TOTA Approved Provider Application. Participation in the TOTA Approved Provider Program or approval of Approved Provider continuing education courses does not imply endorsement of specific course content, products, or clinical procedures by TOTA.
